Objective: The purpose of this study is to investigate factors affecting the resting energy expenditure (REE) of patients with chronic obstructive pulmonary disease (COPD) and interstitial lung disease (ILD), and to create prediction equations. Methods: The present study comprised 32 COPD and 29 ILD patients admitted to the Respiratory Center of Matsuzaka Municipal Hospital. REE was measured using AE-100i (Minato Medical Science, JAPAN) and calculations were performed using the Weir 9s formula. REE was measured 4 or more hours after meals. Evaluated parameters included patient background, pulmonary function test, physical function, body composition, and retrospective analysis of blood biochemical data. For statistical analysis, we used the Pearson9s-moment correlation to evaluate the relationship between REE and other parameters, and multiple regression analysis by the stepwise method. Results: REE in COPD patients was significantly correlated with body weight (r=0.529), predicted BEE (r=0.508), and lean body mass (r=0.565).REE in ILD patients was significantly correlated with body weight (r=0.501), height (r=0.715), lean body mass (r=0.776). In the multiple regression analysis, the lean body mass (β=0.504), WBC (β=0.304) and FEV1% (β=0.378) were independent factors for REE in COPD patients. Lean body mass (β=0.776) was independent factor for REE in ILD patients. The regression model formula for REE in patients with COPD was 467.4 + 19.8 × lean body mass + 4.8 × FEV 1% × 4.8 + 0.022 × WBC and of ILD patients was 345.2 + 27.2 × lean body mass. Conclusion: The results of this study may be useful to improve nutritional therapy of patients with COPD or ILD.
