Burton et al.1 found that thrombolysis at 2 hub centers that mostly used MRI before thrombolysis had lower stroke mimic rates than CT-based thrombolysis at spoke hospitals. They discussed that it is unclear “whether this [finding] can be attributed to the initial clinical evaluation (emergency medicine clinician vs vascular neurologist) or imaging modality,” since the spoke hospital treatments were guided by phone consultations from hub-based vascular neurologists.1
