I agree with Lemmon et al.1 that it is difficult for students to get better at conveying difficult news to patients if students have observed a single encounter. As described in their study, students' desire for repeated experiences of difficult conversations confirms that direct observation in the clinical environment is most beneficial. While beyond the scope of the current study, it would be interesting to know whether participation in similar conversations across a range of specialties cumulatively builds comfort and expertise in the trainee.
