The rapid changes in the field of mechanical thrombectomy (MT) have made patient selection more challenging. Clinical practice is evolving, and treatment is being offered to patients who would not have been enrolled in the existing clinical trials such as those with large vessel occlusion but a low NIH Stroke Scale score, patients with distal vessel occlusions, and patients with concurrent significant penumbra and large core at the time of presentation. How can we best identify those patients who are most likely to benefit from treatment?
