We thank Dr. Berger for the comment on our article1 but do not agree with the hypoxic-ischemic theory. First, the lowest Kirby index recorded in the ICU was 185 mm Hg while the patient was sedated and closely monitored. We recognize that, although significant, it was an isolated determination. Furthermore, it is well known that oxygen cerebral consumption is reduced during deep sedation.2
