We present a video of bladder-sparing laparoscopic pelvic exenteration (PE) using a fascial space priority approach in a 70-year-old male (https://doi.org/10.1245/s 10434-021-11222-4). Computed tomography and magnetic resonance imaging showed apparent rectal carcinoma with prostate infiltration and without distant metastases. Neoadjuvant chemoradiotherapy was initiated and laparoscopic PE with bladder salvage procedure was planned. The operation time was 360 min and the estimated intraoperative blood loss was 50 mL. The postoperative course was uneventful and the patient was discharged on postoperative day 14. Histopathological examination showed all margins to be tumor-free. At the 3-month telephone followup, the patient was satisfied with their urinary function.
