Introduction Olecranon stress fractures (OSFs) are considered a rare overuse injury that primarily affect throwing athletes (1). These injuries are primarily diagnosed in baseball players and Javlineers but have been reported in other sports (1). Pitchers are themost common position in baseball to be at risk for this type of injury (2). Upper extremity stress fractures of any kind are rare (3). OSFs, while not common, are of great concern as they are typically difficult to diagnose. Further, in some instances, they require surgical intervention as opposed to nonoperative management based on the severity of the stress fracture. Furthermore, OSF in the pediatric population (with open physes) present further challenges. Skeletally immature elbows are more susceptible to bony injury given their weak biomechanical structure associated with increased force, such as when throwing or using a weighted ball. In comparison, skeletally mature athletes may likely sustain soft tissue injury (4,5). Thus, timely diagnosis ofOSF is crucial to limit time away from sport and development of a more severe injury. We present a case of a left-handed 15-year-old baseball pitcher with an OSF, the challenges associated with managing this injury, and a focused review of the literature.
