Objective: To develop a standardized process for the administration and documentation of vaccines, in order to improve efficiency and prevent errors. Background: In our clinical experience, there are frequent errors in vaccine administration. Since the majority of our patients get their vaccines from the Vaccines for Children (VFC) program, where supply is determined by utilization, there is no room for documentation error.
