Background: Unplanned extubations in the neonatal intensive care unit cause significant adverse effects on the neonate. In previous studies, reduction of unplanned extubation occurred with the combination of a standardized method of securing the endotracheal tube and, importantly, investment of the staff in making changes. Changes in practice were made using multiple, successive PDSA (Plan-Do-Study-Act) cycles.
