221 tests, including multiple immunofluorescence, ELISA, and immunoblot studies, and a new classification was proposed [2]. This case was diagnosed as LAGBD using this algorithm. LAGBD has been reported to react with various autoantigens, including 120-kDa and 97-kDa LAD-1, laminin-332, laminin1, 6 4 integrin, type VII collagen, BP230, and BP180 [5-7]. Our case showed IgA anti-LAD-1 and IgG anti-BP230 antibodies. The negative immunoblot results using normal human epidermal extract to detect BP230 may be due to its lower sensitivity. Our patient is the first case of LAGBD in the English literature with IgG anti-BP230 and IgA anti-LAD-1 antibodies. One case of erythrodermic LAGBD was reported in the Japanese literature [8] with IgG anti-BP230 antibodies, but antigen for IgA antibodies could not be identified. The pathogenesis of IgG autoantibodies in LAGBD has not been clearly elucidated. Because of the low titre of these antibodies, only anti-BP230 IgG antibodies could be excluded as a cause of erythrodermic lesions in our patient [9]. No LAGBD case similar to our patient has previously been reported and therefore the causative factor(s) for the unique clinical presentation of our case is currently unknown.
