A 16-year-old adolescent girl with no medical history sustained a traumatic, isolated iliotibial band (ITB) rupture near the hip. Examination revealed tenderness over the greater trochanter and pain during resisted hip movements. Magnetic resonance imaging (MRI) showed a 12.8-cm longitudinal partial ITB rupture with intact origin and insertion. Conservative management failed to resolve symptoms. Repeat MRI confirmed persistent rupture. She underwent endoscopic ITB release, resulting in complete symptom resolution and return to sports. Endoscopic ITB release is a viable minimally invasive option for persistent complaints of ITB ruptures unresponsive to conservative management, offering complete symptom resolution and return to activity.
