An 83-year-old man presented with a 4-mm, white, hard, elevated lesion in the upper esophagus that had been discovered on esophagogastroduodenoscopy (Picture 1A). The surface of the lesion was normal, and endoscopic ultrasonography showed that the hyperechoic lesion was located above the submucosal layer with a strong acoustic shadow, diagnosed as a subepithelial lesion (SEL) (Picture 1B). The definitive diagnosis of the tumor was difficult, so we performed endoscopic resection of the SEL for the diagnosis and treatment. Since we deemed underwater endoscopic mucosal resection appropriate, it was used to resect the lesion (Picture 1C, D).
