A 43-year-old man was referred to the emergency room with palpitations and progressive exercise intolerance for 3 months. Laboratory tests showed a high-sensitivity troponin I level of 460.5 ng/l (normal range, 0.0– 34.2 ng/l) and a white blood cell count of 12 000/m (normal range, 4000–10 000/m). Electrocardiography revealed a wide QRS complex tachycardia. Echocardiography showed a left ventricle ejection fraction (LVEF) of 54% with a septal hypokinesia.
