Paravalvular leak (PVL) is a common complication of surgical or percutaneous aortic valve replacement. It may cause heart failure, haemolysis or both and has been shown as an independent risk factor for long-term outcome. Over time, the percutaneous approach has emerged as a reliable and cost-effective alternative to surgery. In this setting, 3D cardiac modelling offers a new way to better understand the complexity and shape of PVLs as well as to test in advance the shape and size of any potential occluding device.
