Introduction Cardiac papillary fibroelastoma (CPFE) is a rare, benign, slow-growing tumour that may have propensity for lifethreatening complications such as thromboembolism, coronary ostia occlusion or mechanical interference with valvular function. CPFEs can potentially affect all cardiac structures covered by endothelium but is predominantly localized on the atrioventricular or semilunar valves with a preference for the left side valves. Histogenesis remains controversial. On the basis of microscopic and developmental features, iatrogenic origin has been suggested. Localization in the left ventricular outflow tract (LVOT) is extremely rare and has never been reported in association with previous chest irradiation.
