In BriefDetection of malignant transformation of low-grade gliomas (LGGs) on conventional MRI is challenging. The authors evaluated the diagnostic accuracy of a single time point dynamic O-(2-[18F]fluoroethyl)-L-tyrosine (FET) PET scanning in a retrospective analysis of 42 pretreated patients with LGGs (WHO grade II) with suspected malignant transformation on MRI. A high diagnostic accuracy (AUC = 0.96) was found only in a patient subpopulation without oligodendroglial components or previous oncological treatment. FET PET imaging alone cannot replace histological confirmation.
