Author's summary There has been an explosion of diagnostic, prognostic, and treatment data in aortic stenosis (AS). Severity of AS should be based not only on valve hemodynamics, but underlying LV dysfunction and comorbidities. Indications for aortic valve replacement continue to evolve, extending to patients with less than severe AS. There are several trials to find a medical therapy to reduce the progression of AS. An application of artificial intelligence and clinical trial results will have a major impact in identifying asymptomatic patients and optimal treatment to the right patient at the right time.
