Quotation marks, a grammatical device for distinguishing text as the words or thoughts of another person, are ubiquitous in society and in medicine. Although there are some good reasons to use quotes in patient medical records, an insidious alternative demands our attention. Quoting patients may imply skepticism or convey disrespect, and we have found that the use of quotes differs by patient race and gender (2). This possible contribution to inequity is one reason clinicians might pause before quoting a patient in a clinical note.
