Abstract: 目的 探讨金属输尿管支架在非肿瘤性狭窄应用中的临床疗效。 方法 采用COOK金属输尿管支架治疗非肿瘤性输尿管狭窄患者,收集患者基本临床资料并记录患者临床随访结果。 结果 对16例结核性顽固性输尿管狭窄患者,6例输尿管激光损伤性狭窄患者,9例结石性输尿管狭窄,2例外伤后输尿管狭窄,2例后腹膜纤维化患者进行金属输尿管金属支架置入治疗,其中16例结核患者在置入3~6个月后均出现严重的输尿管支架结壳现象,均提前拔除。9例结石性患者也不同程度的在1年内出现支架结壳现象,1例患者1年后因结壳拔除困难给予体外冲击波碎石治疗后拔除。2例患者在置入期间出现泌尿系耐药菌感染反复发热给以提前拔除,1例后腹膜患者置入36个月后才给予拔除,支架管表面光滑。而同期肿瘤源性狭窄患者43例患者输尿管支架疗效明显,给予更换12条次支架未见结壳现象发生。 结论 金属输尿管可能不适合结核及结石性输尿管狭窄患者长期留置体内治疗,对于肿瘤源性输尿管顽固性狭窄金属输尿管支架具有良好的扩张引流及支撑作用。
