Abstract: 随着剖宫产手术的开展,近年来膀胱子宫瘘(VUF)的发生率有明显上升趋势,但目前仍缺乏对该病诊治的系统性综述,本文通过回顾国内外各中心对VUF诊治病例的报道,总结VUF的临床表现以及诊治方案的选择。其临床表现可分为三型:I型表现为周期性血尿、无阴道出血、尿液完全可控制,即Youssef's syndrome;II型表现为膀胱及子宫间的双向流动,可见阴道漏尿及周期性血尿;III型表现为正常的月经伴有或不伴有阴道漏尿。关于VUF的诊断,金标准辅助检查仍存在争议,有条件者可同时行MRI及子宫造影、膀胱镜检查。诊断明确后,可根据瘘口的大小及发生时间选择保守治疗或手术干预,其中外科手术修补瘘口是VUF最可靠的治疗方法。 read more here.
