Abstract: 目的 总结经自然腔道应用输尿管软镜技术，采用钬激光为能量平台在处理肾脏囊性疾病中的经验。 方法 2016年12月至2017年10月份共4例确诊肾囊肿的患者纳入分析，均一期置入输尿管内支架管进行输尿管扩张，二期置入输尿管软镜至肾盂、肾盏，镜下寻找囊肿；无法镜下直视发现者，超声多普勒辅助定位，利用钬激光于囊壁中央切开，证实为囊肿后将囊壁呈椭圆形或扇形切除，切缘距离肾实质约0.5～1.0 cm，观察切缘无渗血后，将切除囊壁采用取石网篮取出体外并进行病理活检，退出输尿管软镜，更换为输尿管硬镜放置双J管。 结果 4例均顺利实施手术，3例进镜即发现囊肿；1例初始未发现囊肿，超声多普勒辅助定位后发现囊肿位置，4例均内切开后引流，手术时间27～55 min(平均43.5 min)，术后平均住院时间2 d，术后1个月复查肾脏超声或CT平扫，囊肿体积均明显缩小，术后随访6个月至1年，无复发病例。 结论 在处理肾盂旁囊肿及内生性肾囊肿病例中，经尿道输尿管软镜钬激光内切开术完全经自然腔道，具有创伤小，恢复快，安全性高等优势。 read more here.
