Abstract: 目的 总结经自然腔道应用输尿管软镜技术,采用钬激光为能量平台在处理肾脏囊性疾病中的经验。 方法 2016年12月至2017年10月份共4例确诊肾囊肿的患者纳入分析,均一期置入输尿管内支架管进行输尿管扩张,二期置入输尿管软镜至肾盂、肾盏,镜下寻找囊肿;无法镜下直视发现者,超声多普勒辅助定位,利用钬激光于囊壁中央切开,证实为囊肿后将囊壁呈椭圆形或扇形切除,切缘距离肾实质约0.5~1.0 cm,观察切缘无渗血后,将切除囊壁采用取石网篮取出体外并进行病理活检,退出输尿管软镜,更换为输尿管硬镜放置双J管。 结果 4例均顺利实施手术,3例进镜即发现囊肿;1例初始未发现囊肿,超声多普勒辅助定位后发现囊肿位置,4例均内切开后引流,手术时间27~55 min(平均43.5 min),术后平均住院时间2 d,术后1个月复查肾脏超声或CT平扫,囊肿体积均明显缩小,术后随访6个月至1年,无复发病例。 结论 在处理肾盂旁囊肿及内生性肾囊肿病例中,经尿道输尿管软镜钬激光内切开术完全经自然腔道,具有创伤小,恢复快,安全性高等优势。 read more here.
