Abstract: 症 例 患者:45歳,男性. 主訴:便潜血陽性. 現病歴:検診で便潜血陽性を指摘され,精査目的に 前医で大腸内視鏡検査を施行したところ,盲腸に粘膜 下腫瘍(Submucosal tumor:SMT)を認めた. 2 回の 生検でも確定診断には至らず,精査加療目的に当院紹 介受診となった. 血液生化学所見:腫瘍マーカーを含め,特記すべき 異常所見を認めなかった. 下部消化管内視鏡検査:盲腸に10mm大の黄色調の SMTを認めた(Color 1).頂部の陥凹は,前医で施行 した生検痕と考えられた. 大腸超音波内視鏡検査(EUS):第 2 〜 3 中層に限局 する内部エコー均一な 8 mm大の低エコー腫瘤を認め た(Fig. 1). 治療方針:カルチノイドを疑いboring biopsyを施行 したが,腫瘍の病理組織診断には至らなかった.粘膜 下層に限局する 8 mm大の病変であり,診断的治療目 的に大腸粘膜下層剝離術(Endoscopic… read more here.
