A case of swelling hand caused by ankylosing spondylitis in maintenance hemodialysis

Abstract: 目的 探讨肾内科医师对维持性血液透析患者手肿胀的鉴别诊断水平。 方法 在本院收治1例MHD患者，因强直性脊柱炎外周关节损害引起肿胀手伴疼痛的病例。患者为老年男性规律血液透析，出现左上肢肿胀伴疼痛，以左腕部为重。左腕关节及左掌指关节肿胀、活动受限，局部皮温略高。患者既往强直性脊柱炎病史20余年，目前每日10 mg强的松口服治疗。结合患者病史及辅助检查，考虑为强直性脊柱炎外周关节损害活动导致。 结果 予甲强龙静点抗炎、抑制免疫，联合MTX抑制免疫，同时给予阿法骨化醇、碳酸钙D3抗骨质疏松，另对症加强营养、纠贫、血液透析等综合对症治疗后患者左手关节肿胀、疼痛等症状明显缓解，活动受限好转。 结论 肿胀手综合征是血液透析患者内瘘术后少见的并发症之一，多数由于内瘘侧中心静脉狭窄或者闭塞引起造瘘手的肿胀，但也存在其他疾病所致的可能性，肾内科医师在查找常见病因的同时，要结合患者病史及辅助检查结果，注意鉴别诊断。 read more here.
